Alberta Health Services School Immunization Program will be at Woodman School on June 12th, 2025, to immunize Grade 6, 9, and other eligible students. Children who are eligible to receive immunizations at school will have received an immunization consent package. Please review the information, fill out the consent forms, and return these forms to the school as soon as possible (if you still have the package). If you received a Request for Vaccination Records, please fill out the form and follow the included instructions for providing immunization records to Alberta Health Services. The School Immunization Program follows the Alberta Immunization Schedule.
